Attention all Independent Contractors for Impulse Communications, Inc.
We are required by the Internal Revenue Service to have a form W-9 on file before paying any United States-based Independent Contractors. We ask that all Independent Contractors complete and submit this form to us so that we have it on file. It is important for us to have this information to prevent delays in processing your checks.

In order to make this more convenient for you, we have provided an on-line version of the IRS form so that you may obtain it here. Please follow the instructions below to download the W-9 form.

STEP 1 - The W-9 form has been saved in portable document format (PDF). In order to view the document you will need the Adobe Acrobat Reader. If you already have Adobe Acrobat please proceed to Step 2. Otherwise, you may download it free from Adobe and follow the instructions to install it.

STEP 2 - Next you will need to download the W-9 form and view it with the Acrobat Reader.

STEP 3 - After viewing the W-9 form, please print a hardcopy and complete the form.

• Please fill in your Federal Tax Identification Number, verify your name and address, and state specifically whether you are a Corporation, Individual, LLC, LLP, Not-for-Profit, Partnership, S-Corporation, Sole Proprietor, or Other.


• Be sure that your identification number and name coincide. If you are using a Social Security Number as your Tax Identification Number, then we need your name, not your company name.


• IMPORTANT: You must include your Site Name and Username at the top of the form.

STEP 4 - Once completed, please either mail or fax the form to:
